The benefit will apply to the amounts not subsidized by the beneficiary's health institution (copayment) or to the list value. The conditions that will govern the different benefits will be: 1.-Imaging and Clinical Laboratory (20% discount on co-payment for fonasa and isapres in agreement and 20% discount in list value for individuals and isapres without agreement); 2.- Cardiology and Sports Check up Program (20% discount on co-payment for isapres with an agreement and 20% discount on list value for fonasa, individuals and isapres without an agreement). The following conditions will apply for isapres with an agreement, fonasa and isapres without an agreement: 1.- Physical Evaluation and Sports Consulting (20% discount on list value); 2.- Innovation Center (20% discount on physiology and biomechanics unit); 3.- Footscan and Insoles (20% off list value in evaluation and insoles); 3.- Massage therapy (20% off on relaxation or decontracting massages); 4.- MEDS Healthy (20% discount on list value). a) The benefits and discounts detailed above may be extended, deleted or modified by MEDS, without prior notice. b) In the case of Beneficiaries affiliated with an Isapre, the benefits and discounts will only apply to those Isapres that maintain a current agreement with MEDS for the outpatient setting at the time of requesting the benefit. In the event that the Beneficiary's Isapre does not have a current agreement with MEDS, the benefit will be considered of a “particular” nature. c) The benefits granted under this Agreement are non-transferable to third parties, they are not exchangeable in money nor can they be accumulated with other MEDS promotions, campaigns or discounts. d) Regarding patients affiliated with an Isapre in an agreement or beneficiary of Fonasa, discounts on co-payments, when applicable, will be applied once the issuance of the voucher is verified through the I-MED platform. In the event that at the time of trying to credit the account the bonus for I-MED cannot be obtained, the discount will be applied to the list value.e) Regarding private patients (not affiliated with Isapre or not beneficiaries of Fonasa) The discount will be applied to the list price. Exclusions: The following benefits will be excluded from the agreement: a) Hospital health benefits. b) All those Clinical Laboratory benefits associated with SARS CoV-2 and any benefit associated with said virus that Fonasa incorporates into its tariff. c) Rehabilitation Program Cardiovascular.d) Yoga and Pilates.
